Job Description:
A company is looking for a Care Manager (RN).
Key Responsibilities
Develop and assess complex care management activities for members
with physical needs
Coordinate care plans and collaborate with providers to address
members' unmet needs
Monitor member status and provide education on healthcare options
and resources
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in Rochester, West Long Island, and/or Bronx, NY
