Job Description:
A company is looking for a Transition Care
Coordinator/RN/LCSW/LPC/LPN for a remote position in Virginia.
Key Responsibilities
Collaborate with the Care Coordinator and Interdisciplinary Care
Team to facilitate safe transitions
Assist in managing care for patients during transitions between
different care settings
Engage with facilities and community resources to support patient
needs effectively
Required Qualifications, Training, and Education
Associate's, Bachelor's, or Master's degree in Health & Human
Services or related field
Licensure as RN, LPN, LCSW, or LPC is required
Minimum of two years of Case Management or Long-Term Care
experience
Experience in Nursing, Social Work, or Human Services field
One year of experience working with Medicaid or CCC Plus
populations in Virginia
