We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.Homebase Medical is expanding rapidly into the San Gabriel Valley, CA area. We are currently searching for an experienced full-time Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.

• This position covers patients in and around West Covina, Azusa, Glendora, Arcadia, CA areas in the San Gabriel Valley**We are looking for a full-time provider for 40 hours/week. Any day of the week is open for scheduling.Responsibilities Include
  • Perform primary care in the home setting, Board and Care, and Assisted Living Facilities.
  • Drive to patient locations in your assigned geographic area.
  • Average 7-10 patient visits per day
  • Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
  • Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
  • Spanish speaking a plus
  • Provide referrals to appropriate medical resources.
  • Utilize our EMR system to document patient visit/interaction accurately and timely.
  • Active participation in an on-call rotation is required.
  • Other duties as assigned.Qualifications And Experience
    • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
    • Current, unrestricted PA or NP license to practice in California including DEA license.
    • Minimum of 2 years of experience as an Advanced Practice Provider n primary care, hospice, palliative or urgent care/outpatient setting.
    • Previous experience in a home health, hospice or other home visit setting is highly desired.
    • Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding
    • Great verbal and written communication skills
    • Current drivers' license, auto insurance and reliable transportation
