Job Description:
A company is looking for a Member Health Assessor to join their
team remotely in Indiana.
Key Responsibilities
Participate in the Integrated Care Coordination Team as needed
Coordinate logistics to support care plan goals and
interventions
Conduct assessments by gathering information from members and
stakeholders
Required Qualifications
High School Diploma or General Education Diploma (GED) is
required
Minimum of two (2) years of experience in community settings with
at-risk populations is preferred
