Key Responsibilities
Perform outreach to members to assist with care plans, resources,
and education
Serve as front-line support for member and provider inquiries and
document non-clinical member records
Support member onboarding and administrative duties, including
correspondence and educational materials
Required Qualifications
High School diploma or GED
2 - 4 years of related experience
In-depth knowledge of care management support activities
Experience with member and provider interactions
Ability to comply with state and regulatory requirements
