Job Description:
A company is looking for a Case Manager - RN.
Key Responsibilities
Receives referrals and conducts comprehensive clinical assessments
to identify member needs
Collaborates with members and families to develop and monitor care
plans that promote positive clinical outcomes
Facilitates referrals to community resources and ensures accurate
documentation of member data
Required Qualifications
Registered Nurse with psychiatric experience or a licensed
behavioral health professional
3 years of relevant direct clinical care experience
CCM Certification or ability to obtain within 3 years
Flexibility to work varied hours
Computer application proficiency
